Long-Term Condition Annual Reviews
Our practice invites all patients with long-term conditions for an annual health review. These reviews are an important part of keeping you well, helping us monitor your condition, adjust treatment where needed, and provide support with your ongoing care.
To make the process easier for you, we group all of your long-term conditions into one combined annual review where possible. This means you can attend a single, comprehensive appointment rather than multiple separate bookings.
If you receive a CVD review reminder, you will have one or more of the below conditions;
- – Atrial Fibrilation
- – Coronary Heart Disease
- – Heart Failure
- – Hypertension
- – Non-Diabetic Hyperglycaemia
- – Peripheral Arterial Disease
- – Stroke/ TIA
If you receive a Diabetic review reminder, you will have one or more of the below conditions;
- – Atrial Fibrilation
- – Coronary Heart Disease
- – Diabetes
- – Heart Failure
- – Hypertension
- – Peripheral Arterial Disease
- – Stroke/ TIA
If you receive a Respiratory review reminder, you will have one or more of the below conditions;
- Asthma
- Chronic obstructive pulmonary
We do separate reviews for Learning Disabilities and Mental Health.
You will be contacted each year by text message, email, or letter with an invitation to book your review.
To ensure you receive your reminders promptly, please make sure we have your most up-to-date mobile number and email address. As the NHS App continues to improve, we are increasingly able to communicate securely and efficiently through digital channels, including text messaging. Keeping your contact details current helps us support you better and ensures you do not miss important information about your health.
No appointment booked
If we send you two invitations and you don’t book an appointment we add this message to your record with exception codes for that particular long term condition.
Two invitations sent to patient for a review; no response received and no appointment booked. Patient marked as non-responder for practice processes. Patient may book an appointment at any time. We encourage patient to make an appointment.
