Ordering your prescription

**PATIENT NOTICE**

The online prescription ordering form has been removed as it is no longer supported by our website provider due to its clinical content and the possibility of a clinical safety incident. 

Please see the list below for other ways to order a prescription.

Repeat prescriptions may be given, by agreement with your doctor, for some long term treatments. This allows you to get repeat prescriptions of your medication by the following methods;

  • Posting in a request
  • Leaving the request in the repeat prescription box by the lifts on the ground floor or at reception.
  • Patient Access – see the form below if you would like to sign up
  • By email to [email protected]
  • Unfortunately the practice is UNABLE to accept telephone requests for prescriptions.

REPEAT PRESCRIPTIONS WILL BE READY FOR COLLECTION WITHIN 2 WORKING DAYS OF RECEIVING A REPLY.

You can nominate a local chemist to collect your prescription on your behalf, but they are not allowed to order your medication for you. You need to do this yourself by one of the options listed above.

If you would like to nominate a pharmacy of your choice, you will need to visit them and sign up, we can not do it at the surgery.

The practice prefers to review all patients on long-term medication every 12 months.
