Services

Accessibility

Colne Health Centre has dedicated disabled car parking spaces; access points at the reception desk; disabled toilet and a portable hearing loop. You are also welcome to bring somebody with you to provide support.

If you require assistance on sites or if you have any related information or communication support needs that we can assist with, we ask that you please let us know; your disability may not be visible and we understand that degrees of assistance needed will vary from person to person.

We endeavour to keep a record of any requests made, to support your future visits, however things can and do change so please advise us if the best method of assisting you has altered.

We welcome any suggestions you may have in making your visit to the surgery easier.

Appointments

Many minor illnesses can be managed by over-the-counter remedies and are of short duration. Please can we ask that you have tried to self-manage your illness before you try to book an appointment.

Our phone line can be very busy, please could we politely ask that we prioritise our mornings for patients wanting to make appointments and other queries, like test results be saved for the afternoon when it is less busy.

For our Care Navigators to be able to direct you to the most appropriate clinicians, they need to ask you a few questions about why you are calling us. All information given to our reception or clinical team is treated in confidence.

These are the types of appointments we offer as a practice

 

Urgent on the day appointments

If you need an emergency appointment you will be signposted to the correct clinician such as a GP, Advanced Clinical Practitioner or Paramedic. Your appointment will be a telephone call first and if the clinician wants to see you face to face, they will make you a face to face  appointment.

Emergency appointments are available every day during practice working hours and the amount available depends on how many clinicians are available.

 

Routine Doctor’s appointments

Routine doctor’s appointments are available two week in advance. We have telephone calls and face to face appointments available.

 

Nurse’s appointments

You can book an appointment to see a Nurse 2 weeks in advance. They are face to face appointments but can be a telephone request if needed and appropriate.

 

Long term condition review appointments

All patients with a long-term condition should have an annual Long Term Condition review. The first part of this is usually with a health care assistant who will carry out a few tasks such as taking your blood pressure etc. The second part is usually with a Nurse who will go through your results with you.

Carers

A carer is someone who provides unpaid, often intensive support to family or friends who could not manage without that support. They could be caring for a relative, partner, friend or neighbour who is ill, frail, disabled, or has mental health or substance misuse problems.

Anyone can be a carer and a persons caring role can sometimes last for decades.

Some carers can care for more than one person, some carers have a disability or illness themselves and some have a paid job alongside their caring role. If you are a carer you are just as important as the person you care for. Help and support is available for you!

Many carers don’t see themselves as carers and it takes them an average of two years to acknowledge their role as a carer.

When you inform us you are a carer, we will update your records as soon as possible to show that you are a carer and we will also update the record(s) of the person or people who you are caring for.

Chaperones

You are welcome to have a trusted friend or relative accompany you to your appointment if you wish, to offer moral support or if you feel you might not be able to express what you need clearly, to the doctor or nurse, alone.

If English is not your first language, we can also arrange for an interpreter to accompany you at your appointment; please provide as much notice as possible if this is required.

For intimate examinations, you will be offered the choice of having a trained member of staff present at the examination. Occasionally the clinician may also request this service; this is called chaperoning. It might not always be possible to arrange a staff chaperone at short notice, however if we cannot allocate a chaperone when you or the clinician wish one to be there, we will discuss this with you and can reschedule your examination to another time, if appropriate.

  • Assessment and prescribing of oral emergency contraception, emergency coils need to be fitted at our local family planning clinic.

Health checks

You will be contacted via text / letter or telephone if you are eligible for a Health check.

The check is for people who are aged 40 to 74 who do not have any of the following pre-existing conditions:

  • heart disease
  • chronic kidney disease
  • diabetes
  • high blood pressure (hypertension)
  • atrial fibrillation
  • transient ischaemic attack
  • inherited high cholesterol (familial hypercholesterolemia)
  • heart failure
  • peripheral arterial disease
  • stroke
  • currently being prescribed statins to lower cholesterol
  • previous checks have found that you have a 20% or higher risk of getting cardiovascular disease over the next 10 years

Home visits

Home Visits requests should be reported to the practice via telephone as soon as is possible.

Your request will be triaged and either referred to one of our clinicians or the Acute Visiting Service (AVS)

Hospital visits

If you (or someone you care for) are going to hospital, for any reason, you may benefit from having a Hospital Passport.

Everyone with a learning disability is encouraged to use a Hospital Passport which helps inform hospital staff about the needs of the person with a learning disability and how to support them.

The passport also identifies the person who knows the patient best, such as a carer or a member of the hospital staff; they can provide valuable advice and support in understanding the person’s individual needs.

NHS e-Referral Service

The NHS e-Referral Service (e-RS) provides an easy way for patients to choose their first hospital or clinic appointment with a specialist. Bookings can be made online, using the telephone, or directly in the GP surgery at the time of referral.

Screening

Screening

Screening is a way of finding out if people have a higher chance of having a health problem, so that early treatment can be offered or information given to help them make informed decisions.

If you get a normal result (a screen negative result) after a screening test, this means you are at low risk of having the condition you were screened for. This does not mean you will never develop the condition in the future, just that you are low risk at the moment.

If you have a higher-risk result (a screen positive result), it means you may have the condition that you've been tested for. At this point, you will be offered further tests (called diagnostic tests) to confirm if you have the condition. You can then be offered treatment, advice and support.

Finding out about a problem early can mean that treatment is more effective. However, screening tests are not perfect and they can lead to difficult decisions about having further tests or treatment.

Bowel Cancer Screening

Everyone aged 60 to 74 is offered a bowel cancer screening home test kit every 2 years.

If you're 75 or over, you can ask for a kit every 2 years by phoning the free bowel cancer screening helpline on 0800 707 60 60.

Cervical Screening

Cervical screening is offered to all women and people with a cervix aged 25 to 64 to check the health of cells in the cervix. It is offered every 3 years for those aged 25 to 49, and every 5 years from the ages of 50 to 64.

The screening is quick and painless and can be done here in the practice. Cervical screening is also offered via our extended hours service which means you can obtain an appointment in the evening or at the weekend. Please contact the surgery for more information.

If you are aged over 24 and have never had a smear test, or if it has been more than 3 to 5 years since your last screening, you should arrange an appointment with our Practice Nurse. You should not have the test while you are having a period or in the 4 days before or after your period as this can affect the sample.

Breast Screening

Breast screening is offered to women aged 50 to 70 to detect early signs of breast cancer. Women over 70 can self-refer.

Eligible women will usually receive their first routine invitation for breast cancer screening between the ages of 50 and 53 and will normally be invited every three years until their 71st birthday.

The breast screening programme invites approximately 89,000 women from across East Lancashire every three years. We screen in our mobile screening unit which moves to different locations across East Lancashire enabling women to be screened closer to home. We also screen at our main base at the Edith Watson Unit in Burnley General Hospital.

Abdominal Aortic Screening (AAA)

AAA screening is offered to men during the screening year (1 April to 31 March) that they turn 65 to detect abdominal aortic aneurysms (a dangerous swelling in the aorta). Men over 65 can self-refer.

 

Diabetic Eye Screening

From the age of 12, all people with diabetes are offered an annual diabetic eye test to check for early signs of diabetic retinopathy.

Ways of getting your test results

  • Online - You can view your test results online if you have an online account. If you don’t and would like to see your test results, please click on the link to create an account
  • Telephone - If you have to ring the surgery to enquire about your test results, please can we politely ask that you ring after 2pm, as our reception staff will have more time to deal with your request. This will also free up the phone lines in the mornings, for patients seeking medical help or for appointments
  • Patient Triage – click on this  link or download the NHS app to request your test result. Please note that this is not an urgent service
  • Email – please email your request to admin.p81025@nhs.net Please note that this is not an urgent service

Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate to, unless that person has given prior permission for the release of this data.

When you take your test you will be told how long it will be before the results are returned to the practice. Please do not ring any earlier as this will put added pressure on an already busy phone line.

Travel Vaccinations

Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to fill in a travel assessment form. Once you have filled this in and handed it into the surgery, please give us a call a week later to see which vaccinations you need and if so make an appointment with the nurse.

The form will help the Nurse understand which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to fill in the travel assessment forms as soon as possible- at least 6 weeks before you travel – as an appointment will be required with the practice nurse to actually receive the vaccinations if needed. These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS

If you are planning on travelling to another country it is really important to check the government website for recent travel updates. We would also recommend accessing the foreign and commonwealth website to ensure it is safe to travel to your destination and that you take out comprehensive insurance. It’s important to check with your provider to see whether your policy is covered according to government recommendations.
